Rethinking the healthcare ecosystem through a people-centered model

From Telemedicine to Connected Health, the concept of digital healthcare is constantly evolving, according to a holistic vision based on the centrality of the person.

Digital technologies play a fundamental role in transforming care and health processes, from clinical and diagnostic to organizational and logistical aspects, ensuring flexibility to the system and freeing up resources.

Digital is the enabling element of new healthcare based on data and information, on the interoperability of systems, on services that are usable and accessible to the public in quick times and based on needs.

Thanks to technological coverage in the area of Big Data, IOT, Cloud, Mobile, Cybersecurity, IA and Blockchain, Almaviva proposes a complete offer for the health sector, which is currently complemented by smart working for job security and respect for social distancing.

In Almaviva vision, the public health model must put citizens squarely at the center of the scene and act according to their needs for interaction with the National Health System, along four dimensions, and according to a perspective in which the patient becomes part of their own treatment.

Prevention and lifestyles

Access to healthcare systems

Treatment in specialized centers

Home healthcare follow-up

Almaviva has been working to evolve the healthcare model from hospital-centric to patient-centric, with a focus on prevention, proactive care and better management of chronic diseases, both oncological and age-related.

"Zero-patient" is the goal in this process of reducing hospitalization. This model both prevents the worsening of chronic diseases and deepens our knowledge about the mechanisms and risk factors that lead to the development of these diseases, guaranteeing the population, as far as possible, healthy aging.

The transformation of the model: Decentralized Connected Care

Almaviva’s approach considers the citizen at the center of an ecosystem in which the various resources, intended as structures, services, instrumentation and information are always accessible and interconnected.

A new idea of Connected Health introduces a broader concept of “care” that considers certain new aspects.

Aging of the population and increase of chronic diseases

Promotion of prevention-oriented patient education

Tendency toward delocalization of care, even in the patient's home

Creation of a single digital file of each citizen’s medical history, thanks to the dematerialization of prescriptions and records

Almaviva offering

National Level

Various digitalization projects of the main administrative processes and control for CAP are the object of the solutions such as the ESF, management of the dematerialized prescription managed by the MEF, the prescription fee payments hub using Pago PA, access by means of digital identity recognition – SPID, Health Card and Telemedicine.

Regional Level

Almaviva system integration solutions intended for the regional body tasked with healthcare allow the organization of active services in the territory and their use, through portal and App via SPID, with diversified profiles: patient, healthcare worker, medical and administrative personnel.

Territorial Level

Almaviva solutions are intended to support the Local Health Authority and territorial medicine: solutions of Telemonitoring, Telemedicine, the management of surgical operational centers, the enhancement of patient assignment activities in the territory and home care.

Hospital Level

The solution for the hospital informative system is our ESB integration middleware capable of supporting and creating dialog between all the existing hospital departments in order to standardize the data generated by the various systems. Furthermore, Almaviva Digital Health platform allows numerous diversified services to be activated such as Telemedicine and its various applications (Televisits, consulting, monitoring and rehabilitation), predictive models based on Artificial Intelligence in support of clinical diagnosis, logistics, management control, the creation of anonymized data lakes for clinical and statistical studies.

Big Data and Artificial Intelligence

Based on a social-technological transformation that promotes the continuous collection and analysis of data to generate new knowledge, optimize the allocation of resources and improve organizational processes, Artificial Intelligence systems have now reached a level of technological maturity that allows them to be applied in the healthcare world, always taking into consideration the legal, privacy and ethical implications stemming from their use.

Transforming care systems into a path between automation, integration and regulatory compliance is Almaviva's commitment, which accompanies digital growth in key sectors of the Country.

The experience of the Vimercate hospital has enhanced the use of predictive algorithms in multiple medical branches. Thanks to the implementation of a structured architecture of Artificial Intelligence and Big Data Analytics, powered by the health information heritage of the Healthcare Provider, Almaviva has developed, with a multidisciplinary approach, a range of predictive solutions to support medical and hospital staff in the management of social and health care processes.

Focus on: Blockchain technology to support healthcare data

Since 2017 Almaviva has been implementing solutions for the traceability and immutability of data using Blockchain technology.

Keeping track of the chronology of medical prescriptions, the administration of drugs and the relative assumption of treatments, guaranteeing the information relative to the patient’s consent, the secure exchange of health data, ensuring methods of secure information sharing between healthcare operators and the entities that come into contact with the healthcare structures: the guarantee of data integrity is one of the main advantages of Blockchain in Healthcare.

The definition of a new decentralized, uninterrupted, and comprehensive architectural and interoperability model for the entire health care ecosystem, ensures the most effective way at facing challenges, especially where the coexistence of multiple cooperating realities requires a shared, open and flexible solution involving the collaboration of various actors.
